About our services
There are a range of aged care services available to older people, their families, and carers in Western NSW Local Health District (LHD). These services support Aboriginal people aged 50 and over, and non-Aboriginal people aged 65 and over.
We are home to an ageing population, with almost 1 in 5 Western NSW locals aged 65 and over. People over 70 account for 32% of hospitalisations in Western NSW LHD. Our aged care services support older people to live independently. They also aim to prevent older people from ending up in hospital.
Western NSW LHD is also one of the largest providers of residential aged care in NSW, with more than 400 people living in our 25 Multipurpose Services (MPS).
There are different ways you or your loved ones can access aged care services in Western NSW.
Referring yourself or a loved one
Speak to your general practitioner (GP) or local healthcare team about aged care services that you or a loved one may be eligible for.

Referrals and information for health professionals
Health professionals should use the My Aged Care - Make a referral website to refer patients for an assessment of eligibility to receive Commonwealth Services – this link will take you to a My Aged Care website. All enquiries should go to My Aged Care - Contact us.
GPs can refer patients to the Geriatric Medicine Service online. The Geriatric Medicine Service provides older people with:
- a comprehensive assessment of their needs
- accessing appropriate support services
- emergency respite and health maintenance plans
- managing consultations with specialist physicians in geriatric medicine.
ComPacks provides older people in hospital with community support and case management services.

Hospital clinicians in public and private facilities can refer older patients to the Transitional Aged Care Program (TACP) or Residential Aged Care (RAC). Contact the Aged Care Access Centre for assistance on 1800 94 24 33.
