About our heart (cardiac) care services
If you're living with heart disease or an acute or chronic chronic heart condition, we can help you return to an active, healthy life and work with you to prevent any recurrence of heart symptoms.
Our team will provide you with an individualised education and exercise program, which is tailored to provide support and promote recovery. Our heart health programs will help you:
- take better control of your health
- reduce the risk of further heart problems
- live longer
- learn how to exercise for a healthy heart and get stronger
- feel better, faster recovery - emotionally and physically
- learn how to reduce stress
- become more confident
- return to your former social and work activities.
Our team of heart health professionals can assess your needs and provide specific information to help you manage your chronic heart condition. Your nurse will liaise with your doctor and medical specialist to ensure your care is coordinated and meets your needs. Our staff can provide you with specific and practical information to help you manage your chronic heart condition including:
- recognising and monitoring symptoms
- supporting you with a monitoring app for your condition on your mobile phone
- daily management
- managing your medications and equipment
- preventing and managing symptoms and supporting you with an action plan.
If you have recently experienced hospitalisation for a heart attack, stent, angiogram, or insertion of a pacemaker you may be referred through the Community Care Intake Service or you can refer yourself, to the Acute Heart (Cardiac) Rehabilitation Program.
Our service will assess your needs and provide specific information to help you understand your heart event to ensure optimal health going forward. Your nurse will speak with your doctor and medical specialist to ensure your care is coordinated and meets your needs. Our staff can provide you with practical information to help you understand and manage your heart event including:
- factors impacting a healthy heart
- poor diet
- overweight
- non-compliance with medications
- smoking
- genetic background
- ethnicity
- lack of exercise.
Cardiac rehabilitation usually runs for 8 to 10 weeks. It often starts in hospital and continues when you go home. Programs can be run in different ways to suit your needs, for example: face-to-face, over the phone, in a group or one-on-one and also in your home.

About our lung care services
Our specialist Murrumbidgee LHD lung care team can help you manage your lung condition such as emphysema, asthma or Chronic Obstructive Pulmonary Disease (COPD).
Our Lung (Pulmonary) Rehabilitation Program can help you achieve your personal goals and improve your quality of life by assessing your needs and providing you with specific information to help you manage your lung condition.
Your nurse will liaise with your doctor and medical specialist to ensure your care is coordinated and meets your needs. Our staff can provide you with practical information to help you manage your lung condition including:
- recognising and monitoring symptoms
- supporting you with a monitoring app for your condition on your mobile phone
- daily management
- managing your medications and equipment
- preventing and managing flair ups and supporting you with an action plan.
You can also take part in exercise sessions run by physiotherapists that can help you to manage daily movement and breathlessness.

Do you have asthma and/or have experienced an event during the thunderstorm asthma period September to November?
It's critical that you work with your GP and /or lung specialist to manage and control your condition. They'll provide you with an action plan that is regularly reviewed and ensures you understand the steps that are required when your asthma is flaring up.
Your medications and inhalers are of vital importance, and you should always carry these with you.
If you would like further support with your asthma management, please contact your GP or specialist and arrange a review.
The Asthma Australia information and support centre team provides guidance, information and support and connects you to relevant support services.
You can also contact an Asthma educator on 1800 278 462 (Call 1800 ASTHMA) for free education and support to empower you to better manage your asthma.
