How to apply
When applying for a NSW Companion Card you’ll be assessed against the eligibility criteria.
To be eligible for an NSW Companion Card you must be a person with significant and permanent disability and can provide evidence that:
- You require lifelong attendant care support to access the community
- Aids and other technologies do not help you access the community independently
- You are an Australian citizen or permanent resident and live in NSW.
Note: Eligibility for NDIS does not mean you meet the NSW Companion Card criteria.
- Print, fill and sign your Application Form
- Have the Health Professional Declaration signed by your treating medical or allied health care professional.
- Provide confirmation report/s from your treating medical or allied health service provider to confirm your diagnosis and lifelong support needs.
- Take one, high quality, passport style photo against a neutral background. A digital passport-sized photograph in .jpeg format only. You can use free online tools such as https://www.idphoto4you.com to turn your photographs into passport size.

- Please email all your documents to applications@companioncard.nsw.gov.au
Note: Check the size of your email before sending. You should not exceed 13MB in a single email. Emails with lots of attachments and very large files may not be received, even if you receive an automated response. If you cannot reduce the file size, consider sending your application split across multiple emails.

Review of your application
NSW Companion Card will consider all the information provided with the application when deciding eligibility for a Companion Card. If further information is required, NSW Companion Card will contact the health professional or service provider that signed the application form and/or the applicant.
If your application is approved
If your application is approved you’ll receive a letter with your NSW Companion Card.
If your application is declined
If your application is declined, you’ll be notified by email or a letter. If you would like to discuss this decision you can call NSW Companion Card on 1800 893 044 or email Applications.companioncard@dcj.nsw.gov.au.
If you’re still not satisfied with the outcome of your application you can request a formal review in writing.
