Use this form to lodge a claim for compensation with the Valuer General during a compulsory acquisition.

It is important to note that this form must be completed by both the acquiring authority and you, the claimant.

  • Part A: Completed by the Acquiring Authority.
  • Part B: Also completed by the Acquiring Authority. Ensure it is accurate.
  • Part C: Completed by you, the claimant.
