Adverse events
If there are any negative incidents during clinical trials, whether they involve therapeutic or non-therapeutic goods, it is important to fill out a significant safety issue form in The Research Ethics and Governance Information System (REGIS). Any urgent safety measures that are implemented should be reported within 72 hours of the sponsor becoming aware of the problem. For serious breaches, a serious breach notification form must be completed in REGIS. Read the quick reference guides for instructions.

Complaints and adverse events reporting processes are aligned with:
- section 8 of the Human Research Ethics Committees: Standard operating procedures for NSW public health organisations
