Music festival - early notification form

The Department of Creative Industries, Tourism, Hospitality and Sport will use the information you provide to determine if you need to prepare a health and medical plan to be agreed by NSW Health as a condition to holding your festival.

Complete this music festival early notification form as soon as possible, no later than 120 days before your festival start date.

Has a person been admitted to an intensive care unit (ICU), or required endotracheal intubation, in the last 3 years, or the 3 previous events (whichever is the longest period), because of alcohol or drug use, crowd behaviour or improper safety management:

Your personal information will only be used for the purpose you are providing it in this form. Please read the privacy collection notice for more information about how we handle your information.

You are providing this personal information so that Independent Liquor & Gaming Authority can decide whether your festival requires a health management plan (HMP). Your information will be reviewed by the Music Festivals team in Liquor & Gaming NSW and disclosed to the Secretary of the NSW Ministry of Health and the NSW Commissioner of Police to obtain their advice / collect relevant information pursuant to the Music Festivals Act 2019 (the MFA) to inform Liquor & Gaming NSW decision about the requirement for an HMP.

This information will only be used by Liquor & Gaming NSW to determine if a HMP is required for your festival and for directly related actions. You are providing the information contained in this form voluntarily. If you choose not to provide the requested information, Liquor & Gaming NSW may still determine that your festival requires an agreed HMP. It is an offence under the MFA to hold a festival without an agreed HMP, if directed.

Liquor & Gaming NSW will not disclose your personal information to anybody else (except as noted above) unless it is necessary to prevent or lessen a serious threat to life or health, or it is required or authorised by another law, such as by court order.
