Your information will be reviewed by the Music Festivals team in Liquor & Gaming NSW and may be disclosed to the Secretary of the NSW Ministry of Health, the Commissioner of NSW Police, and the music festivals roundtable to obtain their advice / collect relevant information pursuant to the Music Festivals Act 2019 (the MFA) to inform DCITHS decision about the requirement for an agreed HMP.
This information will only be used by DCITHS to determine if an agreed HMP is required for your festival and for directly related actions. You are providing the information contained in this form voluntarily. If you choose not to provide the requested information, this may delay the time it takes to consider your festival, which may impact whether you can hold your festival. DCITHS may still determine that your festival requires an agreed HMP without the required information. It is an offence under the MFA to hold a festival without an agreed HMP, if directed.
DCITHS will not disclose your personal information to anybody else (except as noted above) unless it is necessary to prevent or lessen a serious threat to life or health, or it is required or authorised by another law, such as by court order.
