Researchers should use this checklist to ensure that all necessary documents are included with a research application. Verifying that all documents on this checklist are included reduces the likelihood that the Research and Information Unit will return the application as incomplete.
Documents that must be submitted include a:
- completed Application for approval to conduct research form
- copy of Participant and or Caregiver Information Sheet(s) and Consent Form(s)
- copy of all research instruments to be used in data collection
- copy of approval from a National Health and Medical Research Council (NHMRC) registered Human Research Ethics Committee (HREC)
- copy of approval from an Aboriginal specific NHMRC-registered HREC
- copy of any other approval granted for the application
- copy of WWCC clearance number and expiry date (where applicable)
- letter to Centre/Community Managers (including a brief project outline).
