About
Remote patient monitoring is a NSW Health virtual care program that uses technology to monitor and safely connect patients with health professionals to deliver care when and where it is needed.
Patients can receive care at home, community or on Country. This helps patients:
- save travel time and money
- get expert care even if they live in a remote area
- connect to a care team that can provide additional support if needed
- boost confidence to manage your health at home.
Program overview
The Remote Patient Monitoring (RPM) Program is delivered by eHealth NSW in partnership with the Agency for Clinical Innovation (ACI). It is funded by the Department of Customer Services’ Digital Restart Fund.
The program aims to deliver foundations for a state-wide remote patient monitoring solution within NSW Health.
How the program works
Patients receive Bluetooth-enabled monitoring devices on loan from NSW Health.
These devices measure the patient’s vital signs, such as oxygen levels, heart rate, blood pressure or weight. The measurements are automatically sent to the patient’s care team via a user-friendly app.
The care team monitors the measurements and may contact the patient if they notice a change in symptoms.
The care team also informs the patient’s GP that they are being monitored virtually. The GP receives a discharge letter upon completion of the patient care.
Eligibility for the program
The program is currently undergoing trial for patients with Congestive Heart Failure (CHF) and Chronic Obstructive Pulmonary Disease (COPD). It may be extended across further patient groups in the future.
Signing up for the program
Patients can ask their treating healthcare professionals or GP about signing up for remote patient monitoring.

NSW Health is required to comply with the Privacy and Personal Information Protection Act (NSW) 1998 and the Health Records and Information Privacy Act (NSW) 2002 in the collection, use and disclosure of your personal information, including health information. NSW Health is also required to keep your information securely stored to prevent unauthorised access.
