We offer a range of mental health services to support older people and people with age-related frailty.
About us
This team provides a range of services to support older people experiencing mental health issues to live and stay well in the community.
Our services include:
- specialist mental health assessment
- care planning
- short and longer term clinical management
- clinical advice to other key services, health professionals and programs
- collaborative activities to support early intervention and recovery.
We work collaboratively with the person to identify ways to improve their wellbeing and quality of life. This may include their ability to live in their own homes, their physical health, social connectedness, housing, education, and employment.
This approach enables the team to provide the appropriate level of care to best support the person during the various stages of recovery.
Referral information
This team is generally for people aged 65 years or older. People under 65 years old, including Aboriginal people aged 50 years and older, can also use this service if it is suitable for their needs.
Referrals can be made through the 24-hour Mental Health Line on 1800 011 511 or through any mental health service within or outside Nepean Blue Mountains LHD, including the Mental Health Triage and Assessment Centre at Nepean Hospital.
Hours of service
Monday to Friday 8:30am to 5pm (closed public holidays).
