Introduction to Health Care Careers application form

We encourage local Year 10 high school students to get a taste for a health care career through the Health Inspirations and Koori Kids Futures introduction to health care careers programs.

Before you apply

Please be aware of the following prior to applying:

  • Only Year 10 students currently attending school and/or residing in the Nepean Blue Mountains Local Health District are eligible to participate. This includes the Penrith, Hawkesbury, Blue Mountains or Lithgow local government areas.
  • Applications will only be considered when submitted by School Career's Advisors.
  • Applications received from outside of the Local Health District will not be considered and will not receive a response from the Committee.
  • Completion of the application form does not guarantee a place in the program.
  • This application will be reviewed by the Committee.
  • Only successful students will be notified in writing about the result of their application throughout the months of May and June 2025.
  • All correspondence will be conducted through School Careers Advisors, Aboriginal Education Officers or School Principals.
  • Successful applicants will be required to:
    • obtain consent from their parent or guardian
    • complete the Health Inspirations attendance form, including media and image consent, medical management plan (if applicable).
    • sign the Student Agreement form

    • complete the Student Placement Record.

       

Health Inspirations and Koori Kids Futures criteria – who can apply

Only students meeting the following criteria can apply for our 2-day Introduction to careers in health care program: 

  • Year 10 high school students who either live or attend school within the Nepean Blue Mountains Local Health District (Penrith, Hawkesbury, Blue Mountains or Lithgow local government areas)
  • Aboriginal and/or Torres Strait Islander and non-Aboriginal or Torres Strait Islander Students.
  • Interested in one or more of the following:
    • Medicine
    • Nursing or Midwifery
    • Psychology
    • Radiography
    • Occupational Therapy
    • Finance
    • Human Resources
    • Communications
    • Information Technology

This form collects personal information, which will be handled and protected appropriately to its sensitivity, and will only be used for the purpose you are providing it in this form. Please read the privacy collection notice for more information about how we handle your information.

You are providing this personal information so that Nepean Blue Mountains Local Health District can process your application.  

This information will only be used for assessing your eligibility to the Health Inspirations and/or Koori Kids Futures work experience program, and for directly related actions arising from that application. It will be stored and protected appropriate to the sensitivity of this information and will not be used for any other purpose.  

Nepean Blue Mountains Local Health District will not share or disclose your personal information to anybody else unless required or authorised by law, such as by court order.  

Personal Information relating to successful candidates will be retained for 7 years or until an individual reaches age 25 (whichever longer) in accordance with GA 28 15.4.4. 

(According to GA 28 15.4.4 only records summarising the employment or service history of personnel. Data includes name, date of birth, dates of employment/service, positions held and salary, locations worked)  

Information relating to unsuccessful candidates will be retained for 2 years per GA28 15.10.1.

Background 

Personal information
