Patients with diabetes can access specialist appointments, tests and procedures without being admitted to hospital.
About us
We provide specialist medical care for people with diabetes to help manage the condition, and screen for and treat complications of diabetes.
Referral information
We see people with a positive diagnosis of diabetes or gestational diabetes, according to the current criteria (ADS Criteria 2015):
- HbA1c > 6.5% (48 mmol/mol)
- fasting glucose > 7.0 mmol/L
- random glucose > 11.1 mmol/L
On a 75g oral glucose tolerance test: fasting glucose > 7.0 mmol/L or 2-hour glucose > 11.1 mmol/L.
In a patient without any symptoms, the test should be repeated to confirm the result and diagnosis. An abnormal result on 2 different diagnostic tests is also acceptable.
The patient needs a written referral from any GP, specialist, or NSW Health service, addressed to the Diabetes Clinic. This referral can be a letter or a diabetes service referral form, providing information about:
- type of diabetes
- diagnostic values (if new diagnosis) or duration of diabetes
- most recent pathology results
- diabetes treatment modality
- relevant medical history and medications list
- current problem
- contact details for the patient and the referee.
GPs and referring clinicians can access the referral form in the Nepean Blue Mountains LHD outpatient referral catalogue.
