Access Application Form

Please complete this form if you want to apply for formal access to government information held by The Cabinet Office under the Government Information (Public Access) Act 2009 (GIPA Act).

The Cabinet Office now accepts payment of the $30 GIPA application fee by credit card. Please contact us on (02) 9228 5871 if you are unable to pay by credit card. 

Before you start your application, please remember

  • You may not need to request a formal access application. The information may already be available or we may be able to give it to you through an informal request. Please contact us or check our Agency Information Guide to find out if the information you want is already available.
  • After you submit the application, you will be emailed a copy of the form. 

What you need

  • Identification documents, such as driver’s licence, Medicare card or passport (if you are seeking personal information).
  • Contact address and phone number.
